The Faisalabad Deputy Commissioner’s assertion that a ten-year-old boy died of cardiac arrest in the Chenab Club’s pool was not substantiated by the findings of the internal inquiry committee.
Formed by the club secretary on Feb 12 to investigate the death of Muhammad Ahmad Saud, a Canadian national of Pakistani origin, at the swimming pool on Feb 6, the committee termed the incident as a “tragic and unfortunate medical emergency.”
However, members also acknowledged “indirect negligence” and a “breach of duty”on the part of the facility’s management.
The inquiry panel comprised Dr. Fraz Saeed Qureshi, Farrukh Ramzan,, Rehan Bashir Kahloon, Dr. Muhammad Hanif, Muhammad Asif Ch, Basim Yaqub, and Usman Zafar, who served as the Convener.
The members were mandated to get to the bottom of the incident and piece together what happened during the incident. They were further tasked with connecting the dots regarding the circumstances leading up to the trouble, specifically focusing on why the CCTV footage had vanished into thin air.
Although the committee was given seven days to submit a comprehensive report, it reached its conclusions only after a considerable delay.
Inquiry Committee Finds Indirect Negligence at Swimming Pool
Wrapping up the report, the committee stated that after detailed deliberations, examination of documentary and electronic evidence, forensic review of CCTV footage and careful consideration of all recorded statements of witnesses and staff members, the committee found indirect negligence, misconduct, or breach of duty on the part of the coaching staff, pool management and operational personnel.
Notably, the swimming pool in-charge was on leave and no substitute was present to look after the affairs in his absence. The remaining staff should have been more vigilant.
Regarding supervision and attendance, the committee noted that based on witness statements, Ahmad Saud was not left unattended at any material time. Staff, club members, and the father of the deceased, Ahmad Saud, were present in the pool area.
The committee found no indication of abandonment or lack of supervision immediately preceding the incident.
Shedding light on the immediate response and rescue efforts, the report mentioned that as soon as the emergency was identified, immediate rescue efforts were undertaken by members and staff present at the swimming pool. The child was promptly taken out of the water without undue delay.
The committee noted that Cardiopulmonary Resuscitation (CPR) was administered without delay by qualified individuals on-site, including medical professionals who arrived shortly to continue resuscitative efforts.
Rescue 1122 was immediately contacted and emergency medical assistance arrived at the earliest possible time under the circumstances. Consequently, Ahmad was shifted for further medical care without unnecessary delay.
Missing CCTV Footage at Chenab Club Pool
Regarding the cause of death, the committee stated: “Based on the available statements of eye witnesses reviewed by the committee, the cause of death cannot be ascertained.”
The committee extended its deepest condolences to the bereaved family, terming the incident as a tragic and unfortunate medical emergency.
The father of the deceased left no stone unturned in his attempt to get the CCTV footage from the swimming pool area; however, he was informed that the footage was missing.
Addressing this disappearance, the committee’s forensic and technical analysis revealed that the loss of footage was due to a power interruption affecting the network switch. This interruption caused a temporary disconnection of the relevant IP camera.
“There is no evidence to suggest intentional deletion, alteration, or manipulation of data from the central recording system.”
Club Offers Independent Forensic Examination
The committee has informed Mr. Saud that if he wishes to pursue an independent forensic examination through any recognized and accredited forum or laboratory, the Club has no objection.
“In the interest of transparency and fairness, the Club is willing to cover the reasonable expenses associated with this process. However, it shall be mandatory for an authorized representative of the Club to remain present during the forensic examination to ensure procedural integrity, proper documentation, and mutual transparency throughout the review process.”
Father Questions Official Claim and Demands Truth
Speaking to Grand Lines, Saud Riaz, a Canadian national of Pakistani origin, commented on the inquiry’s findings. He noted that while the Deputy Commissioner had prematurely attributed his son’s death to cardiac arrest, the committee’s findings suggested otherwise.
He pointed out that cardiac issues are statistically rare in teenagers. He expressed concern that the DC was quick to reach a conclusion, claiming that the club management is attempting to distort the facts rather than uncover the actual cause of death.
Mr. Riaz further said that by sweeping the issue under the rug, the club management is endangering the lives of other children who frequent the swimming pool.
He emphasized that by assisting him in digging out the truth, the club management could establish foolproof arrangements to avoid such tragic incidents in the future.
